Modification of the Single Registration Form in connection with the Amendment to the Trade Licensing Act
The Single Registration Form (SRF) has been designed to be as clear as possible for CRP clients and for subsequent work by CRP staff. The SRF replaces the various types of forms which entrepreneurs had to fill in before starting up their business and then after their establishing themselves at the trade licensing office, the revenue authority, the competent social security administration, the employment office and the health insurance company. The requirements of the individual authorities have been combined into a single form. These requirements are now clearer for entrepreneurs and, at the same time, they no longer have to fill in multiple registration forms demanding the same identification and other information for all the separate authorities.
The Ministry of Industry and Trade, in cooperation with the relevant ministries (the Ministry of Finance, the Ministry of Labour and Social Affairs, the Ministry of Health), has prepared two standard single registration forms — one for legal persons and the other for natural persons. This form can be used for a submission according to the Trade Licensing Act and for a basic registration at the revenue authority, the competent social security administration, the employment office and the health insurance company.

The SRF can be used for the following forms of registration:
1. At the trade licensing office:
Notification of a trade
Licence application
Notification of a change or additional information in accordance with Section 49 or Section 56 of the Trade Licensing Act
Application to cancel a trade authorization
Notification of the commencement/discontinuance of a trade in an establishment
Notification of the interruption of a trade
Notification of the continuation of a trade prior to the end of the period for which a trade has been interrupted
2. At the revenue office:
Tax registration form for natural persons
- ·personal income tax registration form
- ·value added tax registration form
- ·property tax registration form
- ·road tax registration form
- ·cash register registration form
- ·registration form — branches and establishments
- ·income tax registration form for registration as a payer:
- of income tax on emoluments and fringe benefits
- of income tax collected at a special tax rate
- securing tax

3. At the Czech Social Security Administration:
Notification of the commencement of self—employment
Registration form for pension and sickness insurance for self—employed persons
4. At the employment office:
Notification of a vacancy or the filling of a vacancy (for natural and legal persons)
5. At the health insurance company:
Policyholder notification of the commencement (discontinuance) of self—employment
